Sign Waiver and Terms of Play

All league participants must complete a liability waiver to be sanctioned BSSC Members (including substitute players). There are two ways to do so:

(1) Fill Out Your Waiver Online:

Individual Registrants: At the time of your registration, you created an online profile, which required you to submit the waiver form. You have therefore signed a waiver, and no further steps are required.
Team Captains: You must enter your team's roster information into your profile under Roster Management. From the roster information, your teammates will be sent an invitation link to play on your team, and will be asked to complete the waiver online. Please ensure that your entire roster is submitted, so that everyone has a signed waiver online.

(2) Printable Waiver:

In the event that a teammate does not have access to a computer, please print up copies of this waiver for them to sign. Once signed, please mail them to the BSSC office (address listed on the bottom of the waiver).

NOTE: Teams or players who are found with non-sanctioned players will be asked to leave the leagues without refund or warning. Please be advised that we will not share any personal information with other groups or organizations

ASSUMPTION OF RISKS:

I am aware that participating in the activities and sports offered by or associated with, FCSSC, including outdoor and indoor sports such as Ultimate Frisbee, Softball, Flag Football, Basketball, Soccer, Volleyball, Floor Hockey, Curling, Dodgeball, Kickball, Dancing, Golf, Tennis, Downhill Skiing, Snowboarding, X-Country Skiing, Snowshoeing, Orienteering, Canoeing, Kayaking, Mountain Biking, Hiking, Water Polo, Yoga, and Running Groups (“Activities”) exposes me to many inherent risks, dangers and hazards. By engaging in any Activities offered by or associated with FCSSC, I freely accept and fully assume all inherent risks, dangers and hazards and the possibility of personal injury, death, property damage, expense or loss resulting therefrom.

RELEASE OF LIABILITY WAIVER OF CLAIMS & INDEMNITY AGREEMENT:

In consideration of FCSSC permitting me to participate in its activities and sports, permitting me to the use of its equipment and permitting me the use of its facilities, I hereby agree as follows:

TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have FCSSC, and its directors, employees, agents and representatives as a result of my participation in any of the Activities
T0 RELEASE FCSSC, and its directors, officers, employees, agents and representatives from any and all liability for any loss, damage, injury or expense that I may suffer or that my next of kin may suffer, as a result of my participation in Activities offered by FCSSC due to any cause whatsoever INCLUDING NEGLIGENCE, BREACH OF CONTRACT, BREACH OF STATUTORY DUTY OF CARE, AND/OR BREACH OF THE OCCUPIER’S LIABILITY ACT, R S 0 1990, C.O 2 ON THE PART OF FCSSC, or its directors, officers, employees, agents and representatives.

TO HOLD HARMLESS AND INDEMNIFY FCSSC, and its directors, officers, employees, agents and representatives from any and all liability for any property damage or personal injury to any third party, resulting from my activities and my participation in the Activities offered by or associated with FCSSC.
I further state that I am in proper physical condition to participate in the Activities and am aware that participation could, in come circumstances, result in physical injury.
I also give my permission for the free use of my name and image in broadcast, telecast or other media account of the Activities and for the promotional purposes of FCSSC.
That this Agreement shall be effecting and binding upon any heirs, next of kin, executors, administrators and assigns in the event of my death.
I have read and understood this Agreement prior to signing it. I am waiving certain legal rights which I or any heirs, next of kin, executors, administrators and assigns may have against FCSSC, and its directors, officers, employees, agents and representatives.
